Central Oregon Pediatric Associates (COPA) of Bend, Oregon, the largest pediatric practice in the region, is hiring a Full-Time Community Health Worker.

At COPA, the Community Health Worker plays an integral role serving patients at our friendly and welcoming medical clinic for children of all ages. The Community Health Worker (CHW) is an integral member of the Patient-Centered Primary Care Home whose primary role is to connect COPA pediatric patients and families to community social services to improve health equity and outcomes.

The role of the Community Health Worker (CHW) will include but is not limited to the following duties:

Maintains an in-depth knowledge of principles, practices, standards, and techniques and demonstrates knowledge in accordance with COPA policies and procedures within pertinent laws and regulations in the following areas:

  • Screen and assess health status and social determinants of health during appointments and or by PCP referral
  • Assist patients with completion of forms and paperwork
  • Document SDOH screen results in the appropriate electronic tracking systems
  • Provide patient and family navigation to appropriate healthcare, social service systems and other community resources
  • Knowledge of local population demographics, assets, and needs
  • Knowledge of local health disparities and resources to address those disparities
  • Place referrals to appropriate community resources, following up with patients and families on referrals to close the loop
  •  Assist patients and families with scheduling appointments for patients in clinic or other health/social services agency, provide advocacy to address health disparities at the individual and community levels
  • Management of patient information in appropriate computer systems
  • Create and manage connections between vulnerable populations and healthcare providers
  • Provide culturally appropriate health education on topics related to chronic disease prevention, physical activity, and nutrition
  • Advocate for underserved individuals or communities to receive services and resources to address health needs
  • Collect data and relay information to department leadership to inform and improve programs, procedures, and policies
  • Effectively communicate in writing to document patient information and progress
  • Communicate verbally with patients and co-workers in a clear and effective manner
  • Attend and participate in team meetings, communicate with county and community-based agencies/professionals
  • Required travel between COPA clinics, business office and community partners
  •  Follow the COPA privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information
  •  Maintains an in-depth knowledge of principles, practices, standards, and techniques and demonstrates knowledge in accordance with COPA mission, strategic plan, policies and procedures and pertinent laws and regulations
  • Supports COPA and clinic vision, values, and mission
  • Perform other duties as assigned

Qualifications 

Education and Certification:

  • High School diploma or GED required; college level coursework in public health or other healthcare related field preferred.
  • OHA Community Health Worker certification required. Additional certifications recognized in addition to required include: OHA HCI Qualified or Certified Interpreter.

Experience: 

  • Pediatric CHW experience preferred.
  • Exceptional and proven communication skills and ability to interact with a diverse population of professionals. Demonstrates attention to detail and strong time management skills.
  •  Ability to multi-task and work independently. Strong analytical, problem solving and decision-making skills. Ability to work in a team and promote team driven outcomes while also being able to work independently with minimal supervision.
  •  Knowledge of Patient Centered Primary Care Home and Care Coordination curriculum. Clinical informatics knowledge and skills.
  • Epic EMR experience preferred. Proficient with Microsoft Office Suite.
  • Bilingual in Spanish and English a plus.

Physical Requirements:  

  • Use of clear and audible speaking voice and the ability to hear normal speech level in person, over the telephone or through use of other required technology. Sitting or standing for long periods of time. Frequent repetitive motion includes computer keyboard use, reaching with hands and arms, and walking. Occasional lifting and or moving up to 30 pounds and on rare occasion up to 50 pounds. Specific vision abilities required include viewing computer monitor for long periods of time, close vision, distance vision, color vision, peripheral vision, and depth perception.
  • Must be able to wear appropriate personal protective equipment (PPE) as required

 Working Conditions:

  • Standard medical office; may be eligible for combination of on-site and home office. Refer to Local Remote Work Policy for details.
  • Travel between COPA clinics and business office as required.
