The CHW Supervisor provides leadership to the Community Health Worker team across the system, responsibilities include but are not limited to hiring, coaching, training, addressing performance issues, and managing schedules for the team. This role is also expected to act as a Float CHW providing CHW support across the system (all locations where a CHW provides services), a minimum of 50% of time will be dedicated to the supervising and administrative responsibilities.

  • Set weekly Schedule Anywhere schedule and arranges for CHW coverage of pools.
  • Maintain and update CHW Sharesite, reference documents etc. Develop and update training materials and work aids for CHW team use with feedback and input from peers and manager.
  • Manage performance of the team, conduct monthly meetups and provide real-time and consistent coaching with all direct reports.
  • Screen patients for social determinants of health (barriers) and connect patients to available resources

Skills & Knowledge

Strong, proven computer proficiency.   Knowledge of EMR and Excel spreadsheets. Attention to detail. Excellent communication skills. Ability to plan and administer complex work projects and systems.



  • Preferred:Bachelors or better in Social Work.


  • Required: 4 years: Four (4) years experience working in health care or social services. Previous people management experience preferred.
